証明書の発行は、日数を要するものがあります。余裕を持って早めに手続をとってください。
We need several days for issuing certificates. Please allow sufficient time.

自動発券機での申し込み / To apply by automatic publication machine

1. 自動発行機を利用の際は、学生証が必要となります。
A student ID card is necessary to use automatic publication machine.

2. 手数料は、学生証にて納入してください。
Please pay by student ID card.

3. 発行した申請書は、学事課（大学院）に提出してください。
Please submit various applications to graduate school section.

4. 自動発行機は、本館・講義館2階ロビーに設置されています。
Automatic publication machine is installed in the 2nd floor lobby of kougikan and jisshukan.

郵送での申し込み / To apply by mail

「個人情報の保護に関する法律」の施行に伴い、本人確認の手続きをさせていただきますので、証明書の発行に時間がかかることがあります。

1. 手数料は、現金書留又は郵便小為替にて納入してください。
Please pay by registered mail or money postal order.

2. 学生証のコピーを添付してください。
Please attach student ID card.

3. 返信用封筒に切手を貼って同封してください。
Please include stamped, self-addressed envelope.

※電話、FAX、E-メールによる申し込みは、受け付けできませんのでご了承ください。
We do not accept telephone, fax, or e-mail applications.
